I think it would have over 80 licensed cars (the fastest one being a Bugatti if they got the license XD).
It would also have 3 cities which are: Las Vegas (U.S), Birmingham (U.K) and Madrid (Spain)
It would have the same races like checkpoint etc.
Multiplayer would have Cops N Robbers, Capture the Flag, Getaway and much more.
Also a crash course, too.

I don't think they would do Birmingham...
Better to use cities like Sydney, Berlin, Roma, Denver or something like that. However, I think it should have all the original cars from mm1, 2 and 3, but with better graphics and some new cars added. And it should have a "retro-city". That would be the way it is in the mario kart series. They use the best city from an older mm game but they improve the graphics and make the city bigger. Also multiplayer should be improved, there should be traffic!

I agree with Nial. It would be awesome for them to make a remake, with the same cars AND cities, including Chicago and NYC. They should add some new cars, too, like bugatti and Ferrari and Lamborghini, and every major brand that MM2 missed. They should also have Washington DC and Paris, and yeah, maybe Madrid also.

The most important thing- they should make it for PC. It would be good if they make physics more realistic, such as better damage effects, etc, etc. But... probably there won't be something like MM4

If you guys want more realistic physics, try MM1 It looks and feels better, i dont know why nobody plays it :/

I play it Police cars won't follow when the player too is in a police car. Good old days....If you guys want more realistic physics, try MM1 It looks and feels better, i dont know why nobody plays it :/

It's end to itself say what features an hyphotetic next Midtown Madness series game would have, the important is keep the typical feeling of the series, like Midtown Madness 3 kinda succeeded to do IMO. I'm thinking this: if Microsoft will dig up the idea to make a new game of this series, it'll be an exclusive title for the lanch of the next Xbox console (release planned probably for 2013). I would say that's neither 100% sure that it would be called Midtown Madness 4, because the old series reboots or however the use of already used names look to be the normality nowadays, but so far it's especially done by EA Games (e.g.: new SimCity, NFS Hot Pursuit, next NFS Most Wanted, Syndicate; another example is the new Tomb Raider). Agree with Carmaniac, just look at how the racing games are made nowadays, that are less original and there's a less variety of titles released than past.
Unfortunately it's well visible that Microsoft has closed about the PC platform since when it has its console and it's getting success, in the latest years it has releaed for PC only the pretty poor free-to-play Microsoft Flight, minimally paragonable to the old Flight Simulator series.

Windows 98 wasnt failure at all.. it was even more stable than what came out after that (ME and 2000.)
If you would like to see what MM4 might look like, probably it will use that shitty MM3 engine... good on you Microsoft. Or worse, it will be like the Motocross Madness title released not so long ago.
