Usually there shouldn't be problems in adding PKG objects in the .inst file manually through MM2 City Toolkit, it's enough pay attention to the fact that there's only one PSDL terrain block under it and that it has to be the one where the object is connected to. And yes, of course don't forget to manually save the .inst file because the program doesn't really advice of that when closing it.
However, Christoph will give me the files for a check, and probably the problem is something simple.

The .sky file is not present, or it has a wrong file name, or it has a different sky dome name inside.Why there is not sky on it?

There is a file named "hv.sky" and when I open it with word pad or editor, it says "sky_dome 0 0.95 0.005"

Try to generate a new .sky file on MM2CT, or use San Francisco one.

I created a lot of more streets. CityTool says that there are too much polygons to create the map. I shall create a custom map. How can I create a map on my own? I think about the style of the maps of San Francisco or London.

Sad news, but the reason is understandable, and it's not the first time that's happening. Maybe your could at least reduce the city sizes and what you planned to do with it. Almost all the progress there has been in the latest months, should be too early to quit from it.
